The short answer

Chronological age measures time since birth. Bone age compares the maturity of bones seen on an image with a reference method. The two can differ, which may help a clinician interpret growth and development.

Common methods use a radiograph of the left hand and wrist. A trained reader may compare the image with an atlas or score specific bones. Automated systems also exist, but the result still belongs inside a clinical question and a defined method.

Bone age adds context:
it can inform an evaluation or prediction model, but it does not reveal one certain adult height or a guaranteed amount of growth remaining.

What clinicians use it for

  • Adding skeletal-maturity context to an evaluation of unusually slow or fast growth.
  • Investigating differences between chronological development and skeletal maturation.
  • Informing treatment timing in selected pediatric endocrine or orthopedic settings.
  • Contributing to some adult-height prediction methods.

How bone-age assessment can vary

Manual methods have both between-reader and within-reader variability. Reference atlases were built from particular populations and eras, and a method may perform differently across populations or clinical diagnoses. Automated assessment can improve consistency in some settings but does not erase all model and population limits.

The adult-height calculation adds more uncertainty. Published methods can overestimate or underestimate depending on the growth condition, pubertal timing, input quality and prediction model. Even a technically sound result should be presented as an estimate, not an endpoint.

Why a phone photo is not the same test

A surface photo of a hand does not show the bones needed for standard hand-and-wrist radiographic assessment. Cropping an X-ray from a screen can also remove calibration, image quality and clinical context. A consumer upload should not pretend to replace image acquisition, a validated method and qualified interpretation.

What an app may responsibly do

An app can explain the concept, organize a clinician-provided result and show how adding skeletal maturity may change an estimate. It should disclose the source and date, avoid diagnosing from a casual image and keep uncertainty visible.

When to discuss bone age

Bone age is not a routine requirement for every height question. A pediatrician or pediatric endocrinologist can decide whether imaging would add useful information based on the growth pattern, developmental history and clinical examination.
